Actually, based on the two links I will place below, the following seem to be good options:
1) Are you a firearms safety instructor, or been provided with formal training beyond JAMA articles and AMA policy statements? If so please explain your qualifications in this area.

(please note that as a fairly libertarian guy, I have no problem AT ALL with homosexuality, I only use this example because it will likely put the liberal physician on his heels. If anyone has a better “equivalence” scenario, please let me know).
3) Firearms ownership is a civil right, what legal training do you have in civil liberties?
4) Hmmm… I see… well this sounds like a “boundary violation”, perhaps you should rethink your questionnaire in order to avoid future lawsuits.
